E/M Part 1: Levels of Service

In this course, you will discover what constitutes a problem-oriented evaluation and management (E/M) service and how a level can be supported: either on medical decision making or time. The E/M guidelines covered in this course are applicable to ALL E/M services.

Objectives:

Lesson 1: Definitions related to E/M services

  • Identify code categories and subcategories
  • Define new and established patients and initial and subsequent services
  • Name the two options for selecting an E/M level of service

Lesson 2: E/M Key Components

  • Categorize problems as low, moderate, or high severity
  • Apply the data criteria as defined in the guidelines
  • Identify and select the level of risk of patient management
  • Choose a level of service based on medical decision making

Lesson 3: Choosing E/M Levels of Service

  • Pick a level of service based on total time
  • Know when to apply a prolonged services code in addition the the E/M code
  • Determine what supports the substantive portion of a shared service

E/M Part 2: Code Selection

This is the second course in the E/M based series. This course dives deeper into code selection and categories of codes, including guidelines that are specific to each subcategory. Time based services are discussed, as well as modifiers that are commonly used with E/M services.

Objectives:

Lesson 1: Closer look at categories of codes

  • Select appropriate situations to use office or other outpatient services
  • Assess when to use codes for patients admitted and discharged on the same date of service
  • Choose the correct discharge day management code based on documentation

Lesson 2: Time based E/M services

  • Choose prolonged services codes based on time and
  • Name requirements to bill critical care

Lesson 3: Per day codes

  • Determine when to use NICU critical care codes versus intensive care codes

Lesson 4: Modifiers used with E/M services

  • Select appropriate circumstances to apply modifier 24
  • Differentiate when to use modifier 25 vs. 57

E/M Part 3: Care Management and Other Services

In this course, we will look other categories of E/M codes including transitional care management, chronic care management, advance care planning, preventive medicine services and non-face-to-face E/M services.

Objectives:

Lesson 1: Care Management Services

  • Select the components of transitional care management
  • Choose the correct chronic care management codes
  • Identify required elements to report advance care planning
  • Demonstrate understanding of reporting for cognitive assessment and care planning services

Lesson 2: Planning, Assessment, and Intervention Services

  • Determine when to use a preventive medicine services code
  • Differentiate between Medicare’s wellness services
  • Identify when to report both a problem-oriented visit and a preventive services
  • Review different preventive medicine counseling codes
  • Identify the codes used to report behavior change intervention services

Lesson 3: Preventive Services

  • Assign codes for telephone services
  • Identify codes used to report e-visits
  • Determine the requirements for reporting remote physiological monitoring treatment management services

The slides and education on prolonged services was confusing considering there is no longer a time range but a “meet/exceed” requirement and the differing of CPT vs CMS calculations.
