Cardiac Catheterization

This course serves as an introduction to the interventional cardiology series. Beginning with detailed anatomy and physiology, the learner will be presented with clinical information that is integral to the coding process for cardiac catheterization and interventional cardiology procedures. The course continues with instruction on CPT coding for diagnostic cardiac catheterization with tips and tricks to ease the coding process. The course concludes with the Code Red Ultimate Challenge allowing learners to practice and apply their knowledge by coding realistic case scenarios.

Objectives:

Lesson 1: 

  • Identify the branches of the main coronary arteries
  • Label a diagram of the heart chambers and valves
  • Distinguish between native and bypassed coronary vessels

Lesson 2: 

  • Identify procedures that are integral to a diagnostic cardiac catheterization
  • Distinguish between diagnostic right heart catheterization and pulmonary wedge pressure monitoring
  • Select four conditions that are not considered congenital according to CPT when coding for congenital cardiac catheterization procedures
  • Determine the appropriateness of coding peripheral angiography codes performed during a cardiac catheterization encounter

Lesson 3: 

  • Recognize normal vs. abnormal native connections
  • Differentiate between subaortic and subpulmonic circulation
  • Determine which procedures are bundled into codes for congenital heart defect procedures and those that are reported separately

Percutaneous Coronary Interventions

Interventional Cardiology, Part 2 is a continuation of instruction for procedures performed in a cardiac catheterization suite. This course covers primary percutaneous coronary intervention (PCI) procedures including angioplasty, stenting, and atherectomy and details CPT’s progressive hierarchies for coding. It also covers secondary PCI procedures, such as thrombectomy and thrombolysis, and covers other minimally invasive procedures, such as closure of patent ductus arteriosus and transcatheter valve replacements and repairs. The course concludes with the Code Red Ultimate Challenge allowing learners to practice and apply their knowledge by coding realistic case scenarios.

Objectives:

Lesson 1: 

  • Match the major coronary arteries and their branches to their corresponding modifiers
  • Determine when to assign heart catheterization codes in addition to percutaneous coronary intervention (PCI) codes
  • Determine the base code when more than one PCI procedure is performed in a single episode
  • Identify when to use Medicare Level II HCPCS codes for PCI procedures

Lesson 2: 

  • Determine when to use thrombectomy and thrombolysis codes
  • Match approaches for catheter-based aortic valve replacements to their corresponding CPT codes

Electrophysiology

Interventional Cardiology, Part 3 addresses procedures related to heart rhythm. This course covers the important topics of heart rhythm and factors that influence arrhythmias as well electrophysiology studies and ablations and pacemaker and cardioverter defibrillator procedures. As with the other courses in this series, this course concludes with the Code Red Ultimate Challenge allowing learners to practice and apply their knowledge by coding realistic case scenarios.

Objectives:

Lesson 1: 

  • Identify the natural pacemaker of the heart
  • Map the electrical conductivity of the heart
  • Differentiate between types of arrhythmia

Lesson 2: 

  • Recognize the type of equipment used during electrophysiology exam
  • Determine which EP procedures are bundled and which should be coded separately;
  • Identify the components of a comprehensive electrophysiology exam
  • Abstract key documentation points from procedure reports for coding purposes

Lesson 3: 

  • Differentiate between pacemaker and implantable cardioverter defibrillator devices
  • Identify cardiac resynchronization/biventricular devices
  • Distinguish replacement procedures from insertion procedures when devices are replaced or upgraded
