This webinar explores the query process from the perspective of the CDI specialist, focusing on writing clear, non-leading, and compliant queries that align with AHIMA and ACDIS guidelines.
Participants will review when and why to issue queries, who should be queried, and how to construct effective verbal, written, POA, and clinical-validation queries. Through real-world examples and case scenarios, attendees will gain practical strategies to support provider engagement, improve documentation clarity, and reduce documentation-related denials—all while maintaining compliance and professional integrity.
Objectives:
- Describe the role of queries in the clinical documentation integrity (CDI) process and their importance in supporting complete and accurate medical records
- Identify clinical situations that warrant a query, including incomplete, conflicting, ambiguous, or non-specific documentation
- Apply AHIMA and ACDIS query practice guidelines to compose compliant, non-leading queries
- Differentiate among common query types, including verbal, written, yes/no, multiple-choice, POA, and clinical validation queries
- Construct effective and compliant queries using appropriate clinical indicators, treatment plans, and risk factors to support accurate documentation and reduce denial risk

Lisa Romanello, MSHI, BSN,RN, CCDS,CCDS-O
With three decades of experience as a bedside nurse in oncology, orthopedics, neurology, cardiology, and rehab, Lisa transitioned into Clinical Documentation Improvement (CDI) after her nursing career. She served as the manager of a CDI program spanning two Level 1 trauma center campuses, gaining deep expertise in hospital operations and documentation compliance.
Following seven years in hospital CDI management, she moved into consulting, helping organizations nationwide establish new inpatient and outpatient CDI programs, as well as revitalize and expand existing programs. During this time, she earned a Master of Science in Health Informatics, supporting EMR transitions and optimizing clinical workflows.
She has extensive experience providing education for physicians, CDI teams, physician advisors, and coding professionals, combining clinical insight with documentation and compliance expertise.
