Q:  How is preoperative COVID-19 testing coded and does it make a difference if the test is positive or negative?​
A:  Assign codes Z01.812, Encounter for preprocedural laboratory examination, and code Z11.52, Encounter for screening for COVID-19 as noted in Coding Clinic, 2nd Quarter 2023. Effective April 1, 2025, COVID-19 (U07.1) cannot be coded from a test result alone. The coding for preoperative COVID testing is the same, regardless of the test result unless the provider makes a diagnosis of COVID-19.​

Q:  For FY 2024, the Excludes1 notes for categories J43 (Emphysema) and J44 (COPD) were changed to Excludes2. When emphysema with COPD is documented, should both conditions be coded? ​
A:  Not always. When both emphysema and COPD are documented and the COPD is not further specified, assign only a code for the emphysema. When there is an acute exacerbation of COPD or a lower respiratory infection with COPD, codes J44.1 and J44.0, respectively can be coded in addition to a code from category J43.​

Q:  What is the code for non-novel influenza A? ​
A:  Assign code J10.-, Influenza due to other identified influenza virus. Additional characters identify manifestations (e.g., pneumonia, respiratory, or GI). Codes in category J10 describe identified influenza viruses that are not noted in category J09 (e.g., Influenza A/H1N1, avian influenza). Codes in category J11 are assigned when the flu strain is not documented. ​

Q:  How are the hormone receptor status codes used when a patient is triple positive? ​
A:  When ER, PR, and HER2 status are documented, codes in subcategory Z17.4 are used. If both ER and PR are positive, the choice of codes is either Z17.410 (HER2+) or Z17.411 (HER2-). In a patient who is triple positive (ER, PR, and HER2 positive), code Z17.410 is assigned and no codes from subcategories Z17.0-Z17.2 are needed.​
