Q: What is the difference between the new codes 99445 and 99470 when billing for a patient requiring a remote device for physiological monitoring?
A: Code 99470 seen in the previous slides describes the management of a patient, based on the results of physiological monitoring, and can be billed out as a professional service (as well as a facility cost).
Code 99445, however, represents the supply of the device, and this is a facility code only. It has no work RVUs attached to it, so it would not be billed out as a profee service.

Q: My provider does biopsies of the prostate in the office without any imaging guidance. What code would I use to report this in 2026?
A: You could use code 55705, which has been revised for 2026, so it now describes a prostate biopsy when imaging is not done, and is for any sampling method, such as using a needle or a punch biopsy tool.

Q: How can we tell what the information shown in the physician fee schedule’s Relative Value File spreadsheet means?
A: There is a PDF document available on the CMS.gov website that explains how to interpret the Relative Value Files of the Physician Fee Schedule. It is called RVU26A. You can click on the link provided on the References slide (our next slide), then click on the RVU26A link on the website, which will direct you to a list of documents. This list of documents includes the RVU26A PDF and the actual RVU Excel worksheet. Remember, for 2026, there will be two RVU worksheets for the first time – one for qualified providers (QPs) and one for non-qualified providers (non-QPs).
