A: No, patients can only be managed by one provider for chronic care management. Payment will not be made to 2 providers for the same month for CCM services.
Q: Which specialty type is allowed to report CCM services?
A: Typically, primary care providers will report CCM services. Some specialty practitioners may furnish them, but they must be managing at least 2 chronic conditions.
Q: What place of service code should be used for CCM services?
A: The POS on the claim should be the location where the patient would have been seen had services been rendered as a face-to-face encounter.
