Q: Most Local Coverage Determinations (LCDs) state that the primary indication for a facet joint injection is for diagnostic purposes, such as facet syndrome. Is there a way for coders to indicate that the procedure was indeed diagnostic?
A: Yes, Medicare has created modifier ‘KX” for this purpose. Append the KX modifier to the claim line for all diagnostic injections.

Q: How is a dry needle trigger point treatment reported? Is this the same as trigger point injections?
A: A dry needle trigger point procedure and a trigger point injection are similar, but not the same. Trigger point injection procedures will include an injection of a medication into the trigger point.

In a dry needle trigger point procedure, needle(s) are inserted in, or near, the trigger point but there is no injection. The intent is to stimulate the muscle and ultimately lead to muscle relaxation. CPT codes 20560 and 20561 are the codes specific to dry needle trigger point procedures.

Q: In procedure notes for radiofrequency ablation of the sacroiliac joint the procedure note will often mention ablation of the L5 dorsal ramus, S1, S2, and S3 nerves. Code 64625 describes the ablation of nerves innervating the sacroiliac joint. Does this code include all these nerves or is it reported per nerve?
A: Per CPT Assistant, code 64625 includes all the nerves that innervate the sacroiliac joint specifically, which would include nerves from L4 to S2. 64625 would be reported once per side for the entire sacroiliac joint.
