Q: There are CPT codes for imaging of major aortopulmonary collateral arteries (MAPCAs). What are MAPCAs?
A: Major aortopulmonary collateral arteries (MAPCAs) are congenital collateral vessels that connect the aorta to the pulmonary arteries. They develop when pulmonary blood flow is inadequate, which may be due to pulmonary atresia with VSD, or in patients who have Glenn or Fontan circulation.
Q: What is the difference between the Hemi-Fontan and the Glenn procedure?
A: While both the Bidirectional Glenn and Hemi-Fontan are second-stage palliative procedures for children with single-ventricle heart defects, and both share the goal of directing oxygen-poor blood from the upper body directly to the lungs in preparation for the Fontan procedure, they differ in their surgical approach. In a Bidirectional Glenn, the superior vena cava (SVC) is disconnected from the heart and connected directly to the pulmonary artery. In a Hemi-Fontan, the natural connection between the SVC and the heart is preserved, and an internal patch (baffle) is used to redirect SVC blood into the pulmonary arteries. Although the two procedures achieve the same physiologic result and serve the same role in Stage II palliation, the Glenn relies on a direct surgical connection, whereas the Hemi-Fontan preserves more of the heart’s native anatomy and redirects blood flow through an internal pathway.
Q: Which four conditions are not considered to be congenital when coding cardiac catheterization CPT codes?
A: CPT guidelines state that the following conditions are to be coded using the noncongenital cardiac catheterization CPT codes: anomalous coronary arteries arising from the aorta or other coronary arteries, patent foramen ovale (PFO), bicuspid aortic valve, and mitral valve prolapse (MVP).
