Q: Can we bill for shared services in our SNF?
A: Although a SNF is an appropriate setting in which to perform a split/shared E/M service, CMS does state that in a SNF the physician may not delegate a task that the physician must personally perform such as the initial federally mandated comprehensive visit. This service is reported with codes 99304-99306 (POS 31) and may not be reported by an NPP or as a split/shared visit.
Q: Can we bill NICU codes as shared services?
A: No. Codes 99468-99469 are used to report the services of directing the inpatient care of a critically ill neonate or infant 28 days or younger, and only once per day. Per CPT “.. may be reported only by a single individual”.
Q: How do I code a shared service when the NP documents time, but the physician documents MDM
A: Neither CPT nor CMS address this situation, however one MAC (WPS) does suggest that when this happens, to use the MDM to decide the level of service and consider the physician as the billing provider. Please check your local MAC for guidance regarding split/shared visits.
