Q: What determines the correct CPT code for aneurysm repair procedures?
A: The most important factors are:

  • Location of the aneurysm (abdominal, thoracic, iliac, etc.)
  • Type of repair performed (open surgery vs. endovascular)
  • Vessels involved in the treatment

For endovascular procedures, the treatment zone and the type of endograft placed are key elements used to determine the primary CPT code.

Coding Tip: Always identify the approach and the anatomy treated first before selecting a code.

Q: Are catheterization, imaging, and angioplasty coded separately with EVAR or TEVAR procedures?
A: In most cases, no. EVAR and TEVAR codes include many services that would normally be coded separately.

These codes typically include:

  • Nonselective catheterization
  • Radiological supervision and interpretation
  • Endograft deployment
  • Angioplasty or stenting performed within the treatment zone

Coding Tip: Additional codes are usually reported only when procedures occur outside the treatment zone or involve separate access procedures.

Q: How are extension grafts coded during endovascular aneurysm repair?
A: Extension grafts are coded per vessel treated, not per individual device segment. This means that even if multiple extension pieces are placed within the same vessel, the extension add-on code is reported only once for that vessel.

Coding Tip:​ Focus on how many vessels receive extensions, not how many devices were used.
